  • Click on “CDMA” section
  • CDMS section will show the option “Birth/Death certificate- CDMA”, select it
  • Log in to your account
  • Fill your application form completely without any errors and submit it
  • Pay twenty rupees as registration process fee upon completion of your registration

Documents required:

  • If the child is born in hospital or dispensary, then the register record is enough
  • If the birth takes place in the home then approach the concerned local body and inform in writing
  • Parents birth certificates, identity cards and marriage certificates
